Livewell Tier 2 Weight Management Programme
For people with a Body Mass Index (BMI) of 30 or over or 27.5 or over for people from Black and South Asian ethnicity groups. Calculate your BMI here.
Livewell’s 12-month weight management programme focuses on sustainable changes to eating and activity, such as reducing portion sizes, making healthier food swaps, and building regular movement into your day. The programme offers group-based support, free exercise sessions, drop-ins, wellbeing workshops, walking groups and more.

Specialist Weight Management Programme – Tier 3
To progress to Tier 3, you need to have completed Tier 2.
Patients may be referred to Tier 3 if they:
Have a BMI over 50, or
Have a BMI of 35–49 and one or more significant obesity-related health conditions, such as:Type 2 diabetes
Obstructive sleep apnoea
Joint problems
Cardiovascular conditions
Other clinically significant obesity-related conditions
Have complex physical or psychological health needs
Are potentially eligible for Tier 4 (bariatric surgery)
Are unable to access Tier 2 due to neurodiversity, trauma, or other significant barriers
These referrals are considered through regular joint multidisciplinary team (MDT) meetings, often led by psychology teams, to ensure decisions are clinically appropriate.
Tier 3 does not prescribe Mounjaro.
Please note, Livewell does not provide weight loss medication.
Due to funding restrictions, people can only access Livewell’s weight management programmes once.

The NHS Digital Weight Management Programme supports adults living with obesity who also have a diagnosis of diabetes, hypertension or both, to manage their weight and improve their health.
It is a 12-week online behavioural and lifestyle programme that people can access via a smartphone or computer with internet access.
