Freedom of information

Under the Freedom of Information Act 2000, members of the public have the right to request access to recorded information held by public authorities, including GP practices. At Lister House Surgery, we are committed to openness, transparency, and supporting access to information where appropriate.

We publish a range of information proactively on our website, further information can be requested under the FOI Act.

How to make an FOI request

All Freedom of Information requests must be submitted in writing to our quality and patient engagement team:

Please include the following in your request:

  • Your name
  • A description of the information you are seeking
  • How you would like to receive the information (e.g., email)

We will respond to your request within 20 working days, in line with statutory requirements. Some information may be exempt from disclosure, but we will explain this if it applies.
