How to make an FOI request
All Freedom of Information requests must be submitted in writing to our quality and patient engagement team:

Or post a physical request addressed to the the quality and patient engagement team, either posted to the address below, or dropped off at reception at any of our four sites.

- Your name
- A description of the information you are seeking
- How you would like to receive the information (e.g., email)
We will respond to your request within 20 working days, in line with statutory requirements. Some information may be exempt from disclosure, but we will explain this if it applies.
