Vaccinations, travel and injections

Travel vaccines and advice

We offer NHS travel advice and immunisation services.

Please submit a travel assessment form no less than 6 weeks prior to travel. If your planned travel is sooner than 6 weeks you will need to ask for advice via a private travel clinic as the NHS service is not able to provide last minute travel advice.

Submit a Travel Health Assessment here: Travel Health Assessment Form – Fill in form Please complete one form per patient / family member.

The team will then assess the form and decide if you need advice which you will receive via SMS. If you need vaccinations, an appointment will be arranged and an SMS sent to notify you of the site, date and time of the appointment.

If this appointment is not suitable please ring the surgery to rearrange.

Do you need travel vaccinations?

Our nurses can provide comprehensive travel health advice and arrange for appropriate immunisations. Please arrange an appointment for a travel consultation at least 6 weeks before departure if possible. Vaccines need time to take effect and some may require a course over several weeks.

Select the region you are travelling to find out more.

Travel vaccinations and medications

Some travel vaccinations are offered free of charge under the NHS. The following vaccines are available at the surgery:

  • Tetanus
  • Hepatitis A
  • Typhoid

All other travel vaccinations will need to be obtained from and administered by a private travel clinic the surgery does not offer travel vaccinations not covered by the NHS.

All anti-malaria medications are provided privately. We recommend that patients visit a prescribing pharmacy directly who will be able to both prescribe and dispense anti-malarial medication and save the cost of a private prescription from the surgery.

We do not prescribe Diazepam for flying, find out why

Our Primary Care Network Services

Primary Care Network

Primary care networks (PCNs) are a group of GP practices that work together, and other health and care providers, to deliver a wider range of services to the local population that might not be possible within an individual practice. As we have 4 sites and over 45,000 patients, we decided to be our own network, enabling a more streamlined service and efficient ways of working.

‘Team Up’ & Our Housebound population

We ‘Team Up’ with FUSE Health ltd to provide our housebound population with home visits. All visits are triaged by our senior GP and delivered by an advanced practitioner. Housebound patients can call the surgery or submit an online form. All requests will be reviewed on the same day.

Drug and Alcohol services

We refer to Derby Drug & Alcohol Service and even have an in-house clinic to make it easier for patients who live near our Pear Tree site to see a practitioner. This service is available for those with alcohol and drug misuse, medication dependence, who need harm reduction, or who require needle exchange. Please get in touch if you seek support for drug, alcohol, or dependence to prescribed medication.

Smoking Cessation

We can refer into Live Well to help you stop smoking. We also have an in-house stop smoking clinic with a Live Well advisor who speaks Slovak, Czech, and Polish. This is at Lister House Surgery, Pear Tree on a Tuesday afternoon.

Health Kiosk, Blood Pressure & BMI

We have health kiosks at 3 of our sites for patients to use (Pear Tree, Oakwood, Chellaston). You can get a free health check, which looks at body mass index (BMI), blood pressure (BP), and asks you some questions about your health. All results are entered into your health record and will be flagged for clinicians to review if they are above or below our expected thresholds. Our social prescribers also support patients at 10am on Thursdays at our Pear Tree site to use our kiosks and support patients to take ownership of their own health and well-being. Just turn up!

Weight Loss

We can refer into several services such as Live Well, Moorways Sports Centre, and signpost to a variety of local groups and activities.

Social Prescribing (PCN)

What is social prescribing?

Many things can affect your health and wellbeing over time, 1 in 5 GP appointments focus on the wider social aspect of your life rather than acute medical issues such as managing different long term health conditions.

That is where social prescribing fits in and it all starts with a conversation with one of our social prescribers

  • Someone you can to talk to confidentially
  • Someone who is practical and helpful and who will not judge you
  • Someone who can help you decide what you’d like to do to feel healthier or better in yourself
  • Someone who can find you activities that will suit you and, if you want, who can go along with you to them to start with so that you don’t have to go on your own
  • Someone who can advise you on how to arrange appointments with medical services at times that suit you
  • Someone who can give you support along the way.

Our social prescriber can link you with many services in our community that may help you, these may include:

  • Healthy lifestyles and active living
  • Arts, music, outdoor and/or creative classes
  • Befriending, counselling and other support groups
  • Housing, benefits and financial support & advice
  • Employment, training & volunteering
  • Education and learning
  • Getting involved in local groups and activities
  • Accessing specialist services & support that can help
