Top tips for requesting an appointment
Please do…
- Only describe one problem per form
- Check the patient’s details are correct
- Explain the problem clearly and give relevant details
- Add photos if appropriate (e.g. rashes or skin problems)
- Keep your phone with you so we can contact you easily, we will try to call you twice before closing your request
Please don’t…
- Put more than one problem on the same form
- Submit multiple forms about the same problem on the same day
- Use your own details if the form is for someone else (e.g. your child)
- Use rude or inappropriate language
- Send photos of intimate areas unless specifically requested by a clinician

Important to know
- Your form will be saved to your medical record
- We may not always contact you at your preferred time or by your preferred method. This is to make sure that we can offer you the care that you need as efficiently as possible
- If you are added to a waiting list, you will be seen as soon as an appropriate appointment becomes available
Use of Ambeint Scribe (AI) Technology during appointments
Our clinicians may ask you if they can use ‘ambient scribe’ technology to record your consultation using Heidi during your consultation. This will allow them to focus on listening to your concerns without having to type long lists of notes.
Your data will not be stored for any longer than it is needed to enter your medical record and clinicians must check the notes before saving them to ensure they are accurate.
You can decline the use of this technology at any point during your consultation and the clinician will not use it without your explicit consent.

Please keep your phone with you as we may text or call you to offer an appointment, treatment or advice.
Extended hours
We may also offer some appointments during our extended hours.
- Monday to Friday, 6:30pm to 8pm, excluding bank holidays
- Saturday, 9am to 5pm
Cancelling or changing an appointment
Please make every effort to keep your appointment. If you are unable to make it, please cancel it as soon as possible, using one of the methods below, so that it can be reallocated to another patient in need. Your help will avoid a significant waste of NHS resources and is much appreciated.

If you need help when we are closed
If you need medical help now, use NHS 111 online or call 111.
NHS 111 online is for people aged 5 and over. Call 111 if you need help for a child under 5.
Call 999 in a medical or mental health emergency. This is when someone is seriously ill or injured and their life is at risk.
Home visits
Housebound patients who wish to book a home visit should call the surgery to be assessed. Please call us on 01332 271212.
