Requesting Medical Access and Subject Access Requests (SARs)

Due to sustained pressure and excessive levels of demand on the practice, we will only be undertaking the provision of supporting letters for a limited set of criteria. 

Supplying letters is not considered NHS work and is therefore completed in a doctor’s own time, or is in addition to the work our team does after all NHS work is done.

DWP

For the purposes of DWP, we can provide you with a patient summary. A patient summary includes the following:

  • Sensitivities & allergies
  • Latest readings
  • Problems
  • Medication
  • Vaccinations
  • Family history

We will NOT provide letters for:

  • Personal Independence Payment (PIP)
  • Employment Support Allowance (ESA)
  • Universal credit
  • Blue badges
  • Housing
  • Letters for schools
  • Firearms licensing
  • Bus passes
  • Fit to fly
  • Identification
  • Passport suppport
  • Visa support
  • Proof of registration

We may provide letters for other requests

Requests for supporting letters NOT included in the list above will be reviewed on a case by case basis.

You will be contacted to notify you of the acceptance or refusal of your letter request.

It may take up to 8 weeks to process your request from the date of acceptance.

How to request a Medical Report or Letter

To request a medical report or letter, you will need to visit reception any of our four sites and fill in a medical report and letter request form. You will need to bring with you an accepted form of ID as listed below.

  • Passport                                                                                                                            
  • Driving Licence                                                                                        
  • Two Utility Bills (no more than three months old)                               
  • Identity verified by GP/Staff Member
