Patient Feedback Survey

Every time you have an interaction with Lister House Surgery we will send a survey via text message to patients. We do this to learn about patients' experiences and identify what we are doing well and where we can improve our services. The results of this survey are reviewed monthly by colleagues across the Surgery and where feasible, actioned. You can also complete the short service on a computer using the form below.
